In accordance with federal law, all unemployment insurance (UI) benefits are fully taxable and must be reported with a customers annual
tax return.
Each year, the RI Department of Labor and Training mails its customers
an accounting of their UI benefits for the previous year called a 1099G
form. To ensure that the department has up-to-date address information,
please fill out and submit the form below if:
- you have moved since the last time you received unemployment insurance
benefits, or
- if you
believe the Unemployment Insurance Service Center (UISC) may have
an incorrect mailing address for you.
